Swim Social Work Team are looking to recruit a Social Worker to the Hospital Team based in West Yorkshire
This is initially a 3 month full time position with a negotiable salary
You will need to have the following experience
*We require a Qualifed Social Worker to fill a post in our Rapid Response/Intermediate Care.
*This post covers the care beds in 3 care homes in plans to enable safe discharges from Intermediate care either home or in to permanent care.
*You will be the only social worker within the multi disciplinary team.
*The post is full-time.
*Must be a car driver.
*Experience of working in a multi-disciplinary team is preferable.
*The worker will need to carry out assessments and co-ordinate support safeguarding and mental capacity act assessments is required.
You must have
*GSCC Registration
*Recognised Social Work Degree
*Fully Enhanced CRB
