| Description |
Swim Social Work Team are looking to recruit a Qualified Social Worker in the Older Peoples Team based in Norfolk. This is a 3 month, full time position.
Your job will be to:
*With supervision and consultation to manage a caseload as allocated by a Care Manager
Attend case conferences and meetings as requested and produce reports
*Assess individual and carer needs and to commission appropriate services as necessary
*Maintain up to date case records and to provide written reports when required
You must have the following experience:
*At least 3 years post qualification experience
*Previous experience of working in a Older People's Team
You will need the following qualifications:
*Social Work degree / DIPSW
*Full GSCC registration
